Glenn School Authorizations & Release Information

There are 3 mandatory forms on this page that you will need to fill out for each child attending Glenn School with a 4th mandatory for Kindergarten - Owls only. All forms must be completed for each child before the start of the school year.

How to Fill out the Forms Below

Please work your way down this page, filling out each form for your child. Each form is required before the start of the school year. The Field Trip Permission is only mandatory for Kindergarten - Owls only.

If you have multiple children attending Glenn School, for ease of use and to avoid confusion, it is recommended that you submit each form for one child before refreshing this page to begin on your next child.

Each form is independent of each other, so please fill out all the information as completely and accurately as possible for each child. We cannot accept the response “same” on any of the forms. Thank you for your cooperation!

Medical Treatment Authorization

By clicking “accept” below, I grant permission to the Glenn School for Young Children, teachers, and staff, during any emergency or accident involving my child to obtain the services of a physician and/or to transport my child to a hospital if I cannot be reached.

 

Directory Listing Consent

By signing below, I grant permission to the Glenn School for Young Children to list my child and family information in the school directory. This directory will be posted on the password protected section of the Glenn website. Only Glenn School families and staff will have access, and it will not be used for any other purpose other than communication between staff and parents.

 

Photo/Picture Release

By clicking “accept” below, Glenn School for Young Children has permission to publish my child’s likeness on in-house communication. These photos are NOT for social media like Facebook, the Glenn website or other advertising for our school. The pictures will only be shared on platforms that are only shared with currently enrolled families and the Glenn School Board. These include, the weekly newsletter, This Week at Glenn, Class DoJo where pictures are shared only with the class and a few are pulled for the newsletter, Glenn Gala Art Projects, The Glenn School annual yearbook, and displayed on bulletin boards around the school.

Special permission will be obtained for any pictures that would appear on social media, such as our Facebook page or Glenn School website.

 

Field Trip Permission

FOR KINDERGARTEN ONLY. By signing below, my child has permission to go on short walks or supervised field trips with their class and Glenn approved adults during the 2021-2022 school year at Glenn School for Young Children. I understand that I will be notified if the trip involves transportation off the Emory campus. I release Glenn Memorial UMC, Glenn School, its officers, agents, employees, staff, and volunteers from any, and all liability of any kind whatsoever for any loss or injury to my/our child arising from my/our child’s participation in the activities fo Glenn Memorial UMC and the Glenn School.

 

Note: Glenn School for Young Children operates under an exemption from licensing, granted by Bright from the Start. We do carry liability insurance and comply with criteria and requirements for exemptions in Rule 591-1-1-.46(1)(a) Exemption Requirements and Rule 581-1-1-.46(1)(b) Exemption Categories.
